Working Collaboratively

I am a Registered Nurse and I work in Home Health (formally Home and Community Care). My main job is to run an outpatient clinic. The people I serve mainly come in with injuries to their limbs or serious skin infections. One of the first questions I ask is "When was your last tetanus shot?" If they answer is "I don't know" or "Many years ago", I immediately call up one of my Public Health Nurse colleagues. We are conveniently located down the hall from each other. After I explain the situation of my client, they are always ready and willing to immunize my clients after I have treated them. This is a huge benefit to our clients as they can get all their treatment at the same time and at the same place. 

I am proud to be on such a great team who works well together and puts our client's needs first!

Kudos to the Public Health Nurses! Keep up the great work!

I love it when we all work as a team to ensure that our clients are getting the best care possible! I'm so happy to hear that you have made a point of reaching out to those clients that we don't typically see in Public Health and helping to connect us with them to ensure that they receive all interventions available!

A really good point! Health care systems are confusing, complicated and scary when you don't know what you're doing. Having a team the works together to guide you through it makes the experience that much smoother.
